What happens if I can't supply a doctor's note?

My University are asking for a doctor's note, but I'm unable to get one in time for the examining board and they won't consider it if i submit it later. I've sent an extenuating cirstances form and written a letter with explanations etc, but they still want the doctor's note. What happens if I can't submit one?
